The project researchers will ask you to to speak with your doctor about the Preventing Perinatal Depression (PRE-D) Study and whether the PRE-D research team can communicate with your doctor. Please give the following forms to your doctor regarding medication and communication:

• Clinician of Record Agreement
• Practitioner Information
• HIPAA Authorization to Disclose Information
• HIPAA Authorization for Verbal Release of Information
