Research Interests
Chronic kidney disease is associated with increased morbidity and mortality. Diabetes and hypertension are the most common causes of chronic kidney disease. Our team at UT Southwestern is conducting the study Improving Chronic Disease Management with Pieces (IDC-Pieces) in patients with coexistent chronic kidney disease, diabetes, and hypertension. This is a large multicenter pragmatic clinical trial supported by the Healthcare Systems Collaboratory of the National Institutes of Health. Our study is a collaboration with colleagues from the Parkland Center for Clinical Innovations and four large healthcare systems (Parkland Health and Hospital Systems, Texas Health Resources, VA of North Texas Health Care System, and ProHealth of Connecticut). ICD Pieces will test if a collaborative model of care incorporating a novel technology platform and practice facilitators will improve outcomes for patients with chronic kidney disease, diabetes, and hypertension.
